These Terms & Conditions (“Terms”) govern your use of our website and services. By using our website, booking an appointment, or purchasing services, you agree to these Terms.

1) Not Medical Emergency Care

Our services are not emergency medical care. If you have a medical emergency, call 911 or go to the nearest emergency room.

 

2) Informational Use Only (Website)

Website content is for general informational purposes and is not medical advice. Your care plan is individualized and provided during your evaluation and sessions.

 

3) Appointments, Late Arrivals, and Cancellations

  • Please arrive on time for your appointment.

  • Late arrivals may result in a shorter session to respect other clients’ schedules, and the full fee may still apply.

  • Cancellation/No-Show Policy: Cancellations made less than [24] hours before the appointment or no-shows may be charged [a fee or the full session rate]. (Adjust the bracketed items to your preference.)

 

4) Payment Terms

  • Payment is due at the time of service unless otherwise arranged.

  • We may offer packages, memberships, or pre-paid plans. Specific terms will be provided at purchase.

  • Prices and services may change at any time; changes do not affect services already paid for, unless stated.

 

5) Insurance (If Applicable)

If you accept insurance, clients are responsible for understanding their benefits, copays, deductibles, and any non-covered services. If you are out-of-network or self-pay, fees are due as stated.

 

6) Client Responsibilities

You agree to:

  • Provide accurate health and history information

  • Follow safety instructions during sessions

  • Inform us of pain changes, dizziness, or concerns immediately

  • Follow your personalized plan within safe limits

 

7) Assumption of Risk

Physical therapy and movement-based services involve physical activity that may carry risk of soreness or injury. We take reasonable steps to reduce risk, but outcomes cannot be guaranteed.

 

8) Results Not Guaranteed

Each person responds differently to therapy. We do not guarantee outcomes, timelines, or specific performance results.

 

9) Photography/Video (Optional)

We will not record you without your consent. If you consent to photos/video for form analysis or marketing, you may revoke consent in writing at any time.
Default (if you prefer strict privacy): No photos/video without written consent.

 

10) Intellectual Property

All website content (text, graphics, logos) is owned by Flex-Ability Dynamics or licensed to us and may not be copied without permission.

 

11) Third-Party Links

We may link to third-party websites. We do not control or endorse their content and are not responsible for them.

 

12) Limitation of Liability

To the fullest extent allowed by law, Flex-Ability Dynamics is not liable for indirect, incidental, special, or consequential damages arising from use of our website or services.
